Best Headache Tracker App for iPhone
Best for a detailed attack diary with a doctor export: Migraine Buddy. Best for weather and pressure forecasts: Pressure Pal. Best for a migraine disability (MIDAS) score: MiG. Best for logging in one tap with weather, sleep and Watch context filled in: Migraine Headache Tracker Log. Relief is a newer option built around forecasts and reminders. Pick by how much you want to type during an attack.
How we compared. We read each app's App Store listing and pulled rating, rating count and update date on October 12, 2026. We also ran Pressure Pal, Relief and Migraine Buddy's first launch to note sign-up, setup and upgrade screens. We did not subscribe to anything. Disclosure: Jack Wallner makes Migraine Headache Tracker Log, so read its entry with that in mind.

Ratings and update dates are from each app's App Store listing as of October 12, 2026. Apple Watch and iPad show the devices each listing names; "Not listed" means the listing does not name them. Price models describe the structure only, because prices vary by region and change.
What is each headache tracker app best at?
Migraine Buddy keeps the most complete record but needs an account, MiG is the only one with a MIDAS score, Pressure Pal and Relief track weather, and Migraine Headache Tracker Log logs in one tap with no account.
Migraine Buddy
The category leader by rating count. Its listing describes attack records with triggers, symptoms and medications, diary and structured exports for doctors, automatic sleep recording, a 7-day weather forecast and an in-app community. Some features, such as attack duration and coaching, are marked Premium. In our test it opened on a full-screen sign-up and login page with no guest option, so expect to make an account before you log anything.
Migraine Tracker & Diary: MiG
MiG logs frequency, duration, severity, triggers, symptoms and aura, and lists a MIDAS score, a pressure forecast with a sensitivity readout, and a diary export. It is the only app here whose listing mentions the MIDAS questionnaire. The listing shows iPhone only, with no Apple Watch app.
Migraine Tracker: Pressure Pal
Pressure Pal puts each logged attack on a pressure curve, with a 24-hour chart, a 7-day forecast, a medication and trigger log, and a trigger checker that needs a few months of data. Logging is a quick two taps. In our test, a 12-screen quiz and plan screens came before the first log, and the app needs your location for the pressure data to be useful.
Relief - Migraine Tracker
Relief logs intensity, duration, symptoms, medications, weather and sleep, adds custom reminders, and describes AI-assisted pattern detection. It is the newest of the group. In our test an 11-question setup and a plan screen came before the app showed a forecast, so check what is available free before you go through it.
Migraine Headache Tracker Log
Jack Wallner's app. One tap on iPhone, Apple Watch, a widget or the Lock Screen logs a headache and attaches weather and barometric pressure, air quality, and Apple Health data such as sleep and heart rate, with no form. Logging, history, CSV export, widgets and the Watch app are free; the full pattern view, alerts and a PDF for your doctor are in Pro. Limits: it does not record how long an attack lasted, has no medication list, and has only free-text notes for triggers. It has 3 ratings, so its score says little yet.
Which should you pick?
For the most complete record and doctor exports, pick Migraine Buddy. To log fast during an attack with no account, pick Migraine Headache Tracker Log.
- You want the most complete attack record, with symptoms, medications and doctor exports, and you are fine with an account: Migraine Buddy.
- You suspect weather and want pressure forecasts next to your log: Pressure Pal, or MiG if you also want the MIDAS score.
- You want to log in the middle of an attack without typing, and let the app add weather and sleep: Migraine Headache Tracker Log.
- You want to log without making an account: Migraine Headache Tracker Log, Pressure Pal or Relief, all of which let us in without one.
- You track durations and medications closely: choose Migraine Buddy or MiG over a one-tap app.
Frequently asked questions
Which migraine tracker app is best?
It depends on how much detail you want. Migraine Buddy has the most complete attack diary and doctor exports. Pressure Pal and MiG focus on barometric pressure, and MiG adds a MIDAS score. Migraine Headache Tracker Log logs a headache in one tap and fills in weather and Apple Health data automatically.
Can a headache tracker app diagnose migraines?
No. A tracker records when headaches happened and what was going on around them. A clinician makes any diagnosis, and a log can help that conversation.
Do I need an account for a migraine tracker?
Not always. In our test Migraine Buddy opened on a sign-up page, while Pressure Pal, Relief and Migraine Headache Tracker Log let us in without an account.
Are there free migraine tracker apps?
All five apps in this comparison are free to download and offer paid upgrades, so check what stays free before you commit. Prices vary by region.
Is this comparison independent?
Jack Wallner makes Migraine Headache Tracker Log. Competitor entries use their public App Store listings and our own first-launch testing, and the listings are linked below so you can check them.
